We are welcoming new patients! 🙂

  • All providers and locations are accepting new patients
  • Most major insurances are accepted

So for most of you it is simple: go ahead and book an appointment. 

Please bring your completed New Patient Registration Form, photo ID, insurance card, and a list of your current medications to your appointment.

Not sure if your insurance is in-network? Use our Insurance Checker to find out. If you’re not currently in-network, it’s easy to switch over to us!

As a reminder, you can always see any provider at Perlman Clinic.

  • PPO insurance plans: you can see any provider at Perlman Clinic.
  • HMO insurance plans: your insurance card likely lists a Primary Care Provider (PCP). So long as the PCP on your card works at Perlman Clinic, we are a team-based model and you can see any provider at Perlman Clinic.
  • No insurance: you can see any provider at Perlman Clinic.
